The unfortunate reality is that what you're asking for is basically a look at someone's project from start to finish. I'm not sure such examples exist.

Indeed, much of what you're asking for will change depending on the individual's experience. I haven't met anyone who is succeeding on Agile projects doing "agile" by the book.
Having said that, there's lots of guidance out there. Microsoft has some information on agile in their p&p section (http://msdn.microsoft.com
I'd encourage you to connect up with, and follow, the ALT.NET community. They're a little more extreme than most agilistas, but listening to those conversations will give you a good idea of how wide a variety of "agile" is available.
You'll probably want to look at following the blogs of Jeremy Miller, Jeffrey Palermo, David Laribee, Los Techies, JP Boodhoo, Dave Bouwman, and the gang at CodeBetter.com. Udi Dahan is also a great person to keep an eye on, although his stuff is pretty high-level.
In the end, however, your best bet remains finding someone who is successfully "doing agile" and getting them to mentor you.
