where i can get a ready templet of a Help Desk form?

I am managing a university help desk section. I need some help to start working with a paper templet to manage the clients of my help desk request.

Do any one knows or can give an idea of which fields can be included in the help desk form. If there is any ready made form that can be used as an example is welcomed.

You could consider an electronic format using open source software such as Request Tracker (http://bestpractical.com/rt/) or look at the fields incorporated into the software....
