UrlCache MMF Files / Index.DAT Files

Hi, I'm doing some research on how the Index.dat files found on most Windows systems are structured. I've read a few online articles regarding how some folks have reverse engineered them (which have been good articles)  and I've also done a lot of experimenting on my own. In general, I've figured out how "most" of the file works and what information can be retrieved from the different offset addresses. However, some offsets are - extremely - cryptic (probably by design). Does anyone have any information on how these files are actually structured and what all the memory addresses represent, especially in the header?

I'm certain that these files have a detailed "structure" I'm seeking information on what that structure might be.
