Right to the point, some options:
1) Use proxy server. You may want to get your ISP to provide for you. Usually they have proxies.
2) Use anonymous proxy (one of them is like suggested by jlindq above)
3) Use SSH tunnel. This trick requires external server (eg. located at data centre or hosting company). What you need to to is establish an SSH session from your PC to this server, then forward the port you want to use to this server. By doing so, the destination server will 'see' you are coming from this server instead of your original IP.

Google for anonymizers (ie web proxies). Examples can be found through http://www.cexx.org/anony. htm or eg http://www.anonymizer.com/ for web-based anonymizing.
This will only partly hide you. You should also look into a firewall (I'd suggest a standard personal firewall - there are a bunch free ones to download).
