Do you know the TCP port your service offers? There are two health checks you can do, pool and node.
for pool, I go to "local traffic' section, open 'pools'. select your app pool and look under 'health monitors'. there should be a preconfigured 'http' or 'https' check that will try the pool for a '200 OK' status. Click on the appropriate check in 'available' then the << button to move the check to 'active' status and click 'update' below to save.
per-node check under 'local traffic' under 'nodes'. select the node and pick 'node specific' health monitor and follow same instruction as above. click 'update' below to save.
If http/https or other available checks aren't available or correct, you can create your own from 'local traffic' under 'monitors'. you can create tcp, snmp etc. then go select for each pool and node as described above.
