Spyware Driving Me Crazy, ygivui.exe

I seem to be infected with a stubborn spyware program that I can't get rid of.  When I use internet Explorer, multiple IE windows keep opening up.  I've tried all the usual removal routines;  Adaware (with vx2 addin), spybot, spysweeper, Giant Antispyware, hijackthis, dll compare, killbox, etc.  It seems to be caused by a file ygivui.exe.  It is in c:\windows\system32; sometimes it is visible, sometimes not.  I can delete the file and delete all references to it in the registry, but when I reboot, it is back.  I've found triggers for it in MSCONFIG, the Startup folder, and in the registry.  I'm not quite sure why, but it doesn't show up when I scan using Hijackthis, but it does show up in the Hijackthis log.  Also it doesn't show up in running processes using ctrl, alt, del, but it does when I use Process Viewer.  I've copied the lates Hijackthis log below.  Any help is greatly appreciated.
