The project is created, and I am able to commit the files in the root of my project, but when I create a new directory in eclipse, and commit files in that new directory, it fails.

Can you see what happens if you create the same folder with tools outside of eclipse? Is it maybe a problem to add files to the folder as well? This is a weird problem, maybe it has something to do with file system rights. In the example you've given, is root the project folder? Is there not a src folder missing in the path? Should the php file be placed there according PHP coding rules?
Due to lack of info I'll have to ask quite a lot of questions.

With most versioning systems you'll have to create the project folder first (including the path to the project folder). You can use any tool for this really, either a GUI, an explorer (or other folder browser) plugin or - of course - the CVS command tools. From the project folder onwards the folders should be created automatically (of course you can also have the source folder as starting point). So the question is: do you point to a valid point in your CVS tree?
