You should only have to add the line about skipping rbl checks, but you already have that:
skip_rbl_checks 0
Is URlDNSBL checking not working? Look in your /var/log/maillog file to see the results of the scans. You should also raise up the score on those tests to make sure they are high enough to trigger the message as spam:
score URIBL_AB_SURBL 10
score URIBL_JP_SURBL 10
score URIBL_OB_SURBL 10
score URIBL_PH_SURBL 10
score URIBL_SBL 10
score URIBL_SC_SURBL 10
score URIBL_WS_SURBL 10
In my setup if score is 5 or higher it's tagged and 10 or higher it gets rejected.
How are you running SA, as a stand along milter or as part of another package? For MIMEDefang I had to add "$SALocalTestsOnly = 0;" to the config to get it to do the network tests.
