CalmAV + Sanesecurity is very good. I dont find that it cataches the amount of spam that the previous poster sais that his does but it is good at catching the spam which has a tendancy to make it past spamassassin so it is a very worthwhile addition.
For fixing your spamassassin problem there are a few things you can check.
1) Check your spamassassin startup script to see what parameters are being used to start spamassassin. In particular make sure it is not being run in local only mode which disables DNS checks.
2) Make sure that you can resolve DNS queries on your box.
3) Check the logs and make sure the bayes files are being updated. Its a very common problem to have permission issues with these files which can cause them not to be used.
4) Run 'spamassassin --lint -D' and paste the output here. This will list the ddebug output and show which modules are being loaded.

A great way to enhance a SA setup for the type of SPAM you describe, is to use ClamAV + Sanesecurity.
ClamAV is an anti-virus toolkit you can install to process mail *before* it goes through SA. Sanesecurity is a set of signatures for ClamAV that protects against all kinds of marketing SPAM.

We catch about 90% of our SPAM using sanesecurity, only 10% is left to be dealt with by SA.
