Open your Eudora address book and do "File" > "Save As" (type CSV);
you have no choice in formatting, so that's simple :)
Then see "Creating or editing CSV files"

(create headings like those, to match what's in the columns)
Then "Importing CSV files"
http://mail.google.com/sup
You could also try exporting some existing Gmail contacts as CSV,
just to see what sort of column titles Gmail likes.
Your columns to be imported do not need to be in the same order,
because your column titles will tell Gmail which columns mean what,
and Gmail will take care of putting all data where it belongs.
