Reading and Extracting info from Email Headers

I need someone to help me identify some key components of the header.  Especially where it originated from (the name and  the IP address), it's destination (name and the IP address) and the date/time when the email took place.  Someone who can pull as much information from the below header as possible would be nice.  I also want to know why there are multiple IP address in the header and what is really going on here.

Below is the complete header that I have extracted from my Outlook.
