The policy block list mainly contains a list of ip addresses which should not send mail directly aand typically this will be ormal users with dynamic ip addresses.
You said you have your own domain and a fixed IP address. Is yours a business class internet connection or just a home user?
I dont think you can remove yourself from the pbl list. Removal is only really for other lists which contain ip's which have sent spam in the past so removal is for if you had your machine compromised and then could have it emoved after you cleaned it up.
It was probably your isp who supplied the ip's to spamhaus. Maybe you have a residential type service so they say all mail should go through their service.

What I would make sure of is that you do not use Port 25 to send email to your Service Provider, and that you block all Outgoing Port 25 traffic from your Firewall Router, just in case you have Malware on your pc which is spewing out emails on this port.
Are you using a Modem, or are you using a Firewall Router? Using a Modem in conjunction with a software firewall is, in my opinion, insufficient to prevent Malware from sending out on Port 25. This is because the firewall sits on top of the operating system, if the operating system has been subverted then the firewall isn't going to be effective. A Firewall Router gives better protection.
If you make these changes, and you've ticked all the other boxes they have suggested, then you can confidently take the steps necessary to get you off the block list.
