You can back-up Exchange all day, but restoring might be a problem. My company will only support Veritas Backup Exec for Exchange backups. Personally, it's the only backup product I trust. A little sidenote, I was in an SBS 2003 Microsoft training session, during a break, I pulled the Microsoft instructor aside and asked if they had improved NTBackup enough in 2003 to backup Exchange, the instructor lowered his voice and whispered, "still need third-party software".
Microsoft will claim all day it will work, but I've seen too many tragedies to trust it. Same with Arcserve. If anyone has actually ever recovered Exchange after a disaster using NTBackup, please post. I've made this challenge before and got no takers.
So, my recommendation, buy Veritas Backup Exec with the Exchange Agent, don't trust NTBackup.

You need to backup the exchange storage group or groups,
These are included in the backup if you use server 2003 full system backup, You may run into problems if you have the exchange databases on the system drive.
To work around this,
Method 1: Perform Separate Backup Operations
Configure separate backup operations for the System State and the Exchange Server 2003 databases. After the System State backup is complete, the storage group is unfrozen and may be backed up successfully.
Method 2: Move the Exchange Server 2003 Database Files
Move the Exchange Server 2003 databases to a drive other than the operating system drive. This permits you to back up both the Exchange Server 2003 databases and the System State information at the same time.
