With so many solutions, how can you tell which solutions are most likely to help you and which ones are not? How To Disable Browser's Back Button.

Hello EE:
A student needs to logon to my website (http://www.icnasistersinstitute.com) get the course details, lectures etc.

The problem is that once an student is logged out, he/she can use browser back button to go back to the page that should only be accessed via valid login.  Currently, when a user is logged out, i use the following code:

 if (isset($_POST['logout']))
 {
   // Remove session info when a student logsout ...
   unset($_POST);
   setcookie("PHPSESSID","");  // Force session to end
   session_destroy();
 }
I think when the user uses BBB( Browser Back Button), the page is displayed from the cahce.

Question:
(1) Is it possible to enable / disable BBB?
(2) If #1 is not possible, then how can I ensure that the secure pages can only be accessed via valid logged on (instead of using BBB).
(3) How can I make sure that the page is always refreshed from server instead of cahce
(4) Do I need to used session_start in all pages or the index page is sufficient

Sorry for too many question, but I think they are all related.
