When directing a link to a new Firefox window, it often creates that window with different command icons and missing toolbars. I have tried to restore the toolbars via the View->Toolbars menu and the toolbar customization panel, but nothing works. I also noticed that the "theme" of the icons in the navigation toolbar are different than those in the original window (see pictures).

I am using Firefox 3.0.14 with small number of add-ons.
What is the cause of this and can it be fixed?
