If set to automatic, and you're online, pages should refresh as set once you've customized the settings and exited the browser to ensure the settings are in place. If you're interested in synchronizing and setting up pages for offline viewing, this:
To make the current Web page available offline
On the Favorites menu, click Add to Favorites.
Select the Make available offline check box.
To specify a schedule for updating that page, and how much content to download, click Customize.
Follow the instructions on your screen.
Note
Before you go offline, make sure you have the latest version of your pages by clicking the Tools menu and then clicking Synchronize.

Personally, I've chosen to update/refresh each visit to the page, since if I'm not there, I don't want my browser interrupting my other activities unless and until I wish to view the page. All end-user preferences, of course.

Within IE - tools - internet option - Connections - check your setup options .... you can choose as I do, to refresh web pages every visit to the page, or perhaps as you note above, change it to automatic.
WindowsUpdate has patches and fixes, depending on your version of IE. The most current is IE 5.5 with service patch 1.
