I am only using Leopard and Safari so whatever comes with those.....
No third party software relevant to firewalls etc....

I have in the safari appearance preferences "display images when the page opens" ticked already.

I have Parallel's on the Mac which has AVG firewall....
I also have AVG installed on a laptop which is on my network.......
I access the internet via a wireless modem/router.........ADSL 2+.
The question marks are random...I can't share the site it happened at last time as it was a "Payment Receipt" screen where the companies logo's and other pictures were all question marks....
The example you give is a good illustration of what may be the problem ... many shopping websites use HTTPS shopping carts but include links to images/graphics from their standard website which in NOT HTTPS Secure. Browsers, including Safari and others are often reluctant to display NON-SECURE graphics or content when viewing a SECURE website page.
Have you noticed this happening on any NON-SECURE websites? Otherwise I think you may have discovered a security "feature" of modern browsers.
Look at the source code for the page and you should see the path to the images ... if they are prefixed with http:// or are located on a website different to the HTTPS page then Safari etc do not like to display them.
Nope .. the Pop-up windows is a different thing .. does what it says really.
Have you enabled - "Private Browsing" by any chance??

The blue question mark implies that the download of images was either stopped or blocked.
1. Stopped - when you cancel the downloading of a page before all the images have fully loaded.. any which are incomplete will appear with a blue question mark.
2. If you have any firewall or blocking software running on your computer. Certain graphics - such as those which are not on the originating website may be blocked? Are you running any such software?
