Out of Office enabled but not sending

Out of Office is enabled for a mailbox, however no out of office message is generated for messages received.  The out of office setup has been tested from  OWA and Outlook client, a new profile, mailbox has even been moved to a different server to ensure no corruptions, and a profile even tried from another machine.  Any ideas?  versions are:  Outlook 2003 / Exchange 2003Start Free Trial
