The icon is there and the message gets sent (appears in sent items). Sometimes it does, sometimes it doesn't. The same thing happens when replying to a message.
This problem happened after migrating the user to a new server.

If you access the mailbox from another computer, do you see the message status information correctly.
Do you have any firewalls between the user (including on the PC) and the server that may prevent full communication with the server?
Depending on your configuration, some of the necessary ports for Exchange Client/Server communication are:
TCP 110 (POP3)
TCP 143 (IMAP)
TCP 389 (LDAP)
TCP 390 (Alternate LDAP, not typical)
TCP 636 (LDAP over SSL)
TCP 993 (IMAP over SSL)
TCP 995 (POP3 over SSL
TCP 3268 (Global Catalog)
TCP 3269 (Global Catalog over SSL)
NNTP, HTTP, and SMTP would not be at play for your issue.
No there is no firewall between the user and the server, and yes the user is in cache mode.
The user has logged in both the OWA and on Outlook on another machine, but messages are still not be tagged.
I have read the following artcile on the Microsofthelp site, and it says to Configure Outlook to run in Cached Exchange Mode, however the user is already in cache mode, and this article applies to Outlook 2003 only.

Did you create new profile in Outlook or modify the existing profile? Is the user in Cache mode?
