Using outlook 2003, I want to deploy a custom appointment form

I have a custom outlook 2003 appointment form, running on vbscript.

The requirement is to deploy this form to a large number of users in a domain and make it EXTREMELY easy to open.
How do you add a new button/toolbar inside of outlook that launches the form, and deploy this to multiple machines?

If it helps, I can program in vbScript.
