Here is a person who had the same problem and said that the problem went away when they started using R5.0.5 to send the stored form:

We have recently upgraded to Notes 6.0
When I open a document in the database, I get the following message twice "A stored form can not contain computed sub forms".
I can still get to the document if I just click OK both times.
This was not a problem with the earlier versions.
Does anyone know how to stop this? and why this is happening?

Yes, I've seen that. This document was mailed, correct? The existing documents can't easily be fixed, but you can prevent this in the future. Change the computed subform as follows:
Let's say the computed subform code was as follows:
isUserAdmin := @UserRoles = "[Admin]";
isUserExec := @UserRoles = "[Exec]";
@If(isUserAdmin; "DebugShowAll"; isUserExec; "ExecSummary"; "NormalFields");
Change it to:
isUserAdmin := @UserRoles = "[Admin]";
isUserExec := @UserRoles = "[Exec]";
subForm := @If(isUserAdmin; "DebugShowAll"; isUserExec; "ExecSummary"; "NormalFields");
