VBA - Rearrange Data on one worksheet into a different order on a second worksheet

How are you doing?

I need a macro to do the following (please see attached spreadsheet)

I have data in columns A:D, but the issue is that there is only one entry for A and B, (ie A4:B4) and then columns C and D have entries going down C4:C6 and D4:D6. What I want to do is put this all on one row, so that it goes like this.

On the output sheet:
A2 and B2, would be copied. D2 would go to C2, D3 would go to D2 and D4 would go to E2.
Next row (starting at A3)

Hopefully the attached spreadsheet explains this in a bit clearer manner!
