=SUM(A1,B1,D2) & " see note at bottom"

If you want to fix the format of the numeric part, e.g. to display a fixed number of decimal places you might want to use FIXED or TEXT function to ensure that, e.g. for Dave's example, if you want the sum always to display to 2 decimal places
=FIXED(SUM(A1,B1,D2)
