Access/Excel VBA Userforms - Search and Calculate

I have 3 pools of information:

Calls Answered Within 30 Secs
Calls Abandoned Within 30 Secs
Total Demand

With the above information I'd like help in creating a userform in either Excel VBA or Access in order for the user to input dates into combo boxes (date from and date to) which would then run a formula (Calls Answered Within 30 Secs + Calls Abandoned Within 30 Secs / Total Demand) in order to show the service level (as a percentage) achieved between the dates selected by the user.

At present the historical data is stored on an Excel worksheet but there wouldn't be an issue transferring to Access if it made things easier.

Any help, advice, links etc would be massive helpl as my knowledge is limited and I just can't seem to get off the ground with this one.
