Your question could fill a book; Look in the help system for help with tables of contents and indexes; Were you to read the manual you would find that you do not need to manually index page numbers, indesign will do that. The help system/manual can be found under the help menu listed as indesign help. I suggest you search for the word index and read those topics. To insert an index entry use the keyboard shortcut ctrl+u.
Yes, indesign has anchored frames, found on the insert menu as object > anchored object > insert; This has nothing to do with indexing though;

To clarify my question:
We know where we want to insert this "anchor page reference" in out XML. We then want to use these anchors to build the index and have the index automaticly update the page number references as things are pushed back and forth due to manual editing. Is there such an anchor in Indesign at all? We've tried to search for help on the topic but as always with Indesign it's not easy to find what you're looking for.
