Any other programs that enable easy updates - where a new item can be inserted, shiting the existing items forward?

depends on your knowledge of databases and what you want out of indesign. XML based imports can produce absolutely massive catalogues without the need for plug-ins, but it requires a bit of up-front knowledge. i'd recommend purchasing a book called "Designer's Guide to Adobe InDesign and XML, Harness the Power of XML to Automate your Print and Web Workflows" and there is a specific chapter (chapter 7) and my work has made several proof-of-principle merges using third party generated XML.

personally, i'd use datamerge first if the data wasn't too complicated, but if the data had to be like a phone book (flowing from entry to entry rather than page to page like a business card merge) i'd use XML solutions which are available.
Adobe has just released a new publishing program called ROME. I downloaded it the other day and haven't used it much. It does have multi page abilities. As far as inserting new pages, it has this function. You will be asked where to insert the page, when you do it will drop all pages past it down.

I would recommend InDesign, but I don't know about using the plug in. I am old school and would rather have complete control myself.
