Here is a way to make it work. Under File, go to page setup menu.
Select:
Layout Settings tab:
Set “Plot Area” to ‘extents’
Set “Plot Scale” to ‘scaled to fit’
Now, if you have a border that fits your A0 or A1 paper size, and have these settings, the PDF file will scale your border, i.e. the extents of your dwg to fit onto the PDF file.
This is what we do, simply scale borders, and print extents, rather than scaling parts on drawings. That way, all the parts are drawn 1:1 and it is just the border that is scaled.
Also, acad has other paper sizes in the plot scale that you can select prior to printing the pdf file. Try both, but the first explnation should work just fine.

Not sure how to print out of AutoCad but into the PDF Writer is as follows...
FILE - PRINT - change printer to Adobe PDF - LAYOUT - ADVANCED - Paper Size: PostScript Custom Page Size and enter the desired Width x Heigth
