I posted the question more than a week ago, and while I do appreciate your help, I've already solved the problem. I've also posted the question on many sites, so I apologize if I neglected to post the solution here as well.
There was no problem with the javascript or the MIME type association for FLV on the server and the swf file was published with the proper security model.
The problem was that I was using a .SMIL xml file to list the flv files (only one at the moment) I wanted to play with the FLVPlayback component. However, the SMIL extension did NOT have a MIME type association. As soon as that was added, everything worked perfectly.
The MIME association for SMIL is [application/smil-xml].
I will issue a request to PAQ this question.
