Receiving/Sending Multiple copy of same email in differerent days

We are using Exchange 5 Server.
Recently, some of our staff complaining either (1)receiving same email from outside company in this serveral days (same email, but sent 3 - 12 times across 3 - 6 days). or (2) Sending our multiple content to outside client.

The email content is real content, not the "How are you" virus...

This is Virus, What virus would do this? We scaned our computer and server but found nothing...

What else can this be?
