Strange, intermittant calendar problems with delegates and Outlook 2003

We have been having some real problems with Exchange 2003 - specifically with four people being delegates for the CEO and working on his calendar.  Every once in a while, an appointment that someone swears they put on his calendar doesn't appear.  Today, a number of appointments that were created or changed on Friday of last week either disappeared or reverted to their pre-change condition.  

I know that this is a bit of a vague question, but I'd love some suggestions as to where I can check to make sure that everything is set up correctly, either in Outlook or on the Exchange server.

Running:
Exchange 2003 with all patches as of today installed
Windows Server 2003
About 40 users total in a single AD forest
Lots of mail transactions and calendar appointments - CEO usually has 15 or more appointments per day
Each admin is a delegate with access to the CEO's calendar, and they send out meeting invitations from their own outlook, using his calendar (so the e-mail says from A on behalf of B)Start Free Trial
