Delivery of External Read Receipts
I seem to recall that there is a place that you have to tell Exchange that you allow the delivery of read receipts to the outside world, am I mistaken? If I am correct, where is this setting?

yes and no, there is not a setting that directly relates just to read receipts on exchange 2003
The closest you can get at a server level is in ESM, Internet Message Formats, right click on the Default and choose Properties. Then choose "Advanced" and de-select "Allow Delivery Reports", that will stop read reciepts been able to be sent out, selected box will allow it - but that only deals with receipts that go through SMTP
the actually setting for read receipts are set at the client level. you can turn this off and on via group policy if needed.
