I had run the ISINTEG command after repairing the databases initially (had to delete the transaction logs to get this to work) and was having the issue. After the server was up for a while and had new transaction logs I ran it again, after which the calendar items worked.

Have you run ISINTEG command after repairing the database (eseutil /p) and offline defrag (eseutil /d)?
