What we're looking to do is add a couple of forms (or is template the correct term?) to the standard forms library and then give the users easy access to those forms either as an option under the 'New' pull down menu or by adding them to the toolbar under a 'My Forms' category.
The forms/templates would be used to submit customer requests to our help-desk. Routing would go from the requester to their manager. If the manager approves the request, the form would then go to our help desk (helpdesk@xxxxxxx.com). if the manager denies the request, the form would go back to the requester along with any comments the manager wanted to include.
We support restaurants, so the requests would be of two types: POS changes (Add/Change/Remove buttons on the registers) or Product changes (A/C/R liquor,wine,beer or food items). We need a form for both types.
My in-house tech has created the forms but isn't sure how we go about adding them to the toolbar or to the 'New' menu or how to setup the routing.
Ideally, we'd like to add the forms to the Exchange server so they are available to everyone without us having to modify each Outlook client individually.
Does any of that clarify the issue?

I think you need to create a html form, might be easier to plop it up on the intranet, rather than modify outlook, when forms are accessible to outlook, outlook will provide a lookup tool anyway. I would suggest to group them logically, and I would try to stick to some naming convention if possible..
