Plesk / Qmail cannot send mail from PHP
I have a customer who is using PHP (phpmailer: http://phpmailer.sourcefor
If they use an alternate server (comcast) with authentication, the emails appear to go out just fine.
Why arent' these getting sent from the mail server?

Check if the sendmail-binary is located at /usr/sbin/sendmail (that the default path used in phpmailer) and is useable. Also check the mail-server logs if mails have been queued or not.
