Dynamically populate Select dropdown boxes using CF and JavaScript

I have a page that needs to populate drop-down select boxes dynamically.
It currently works at a 2-tier level, but I need it to work at a 3-tier level.
Below is the code I use so far. Please could someone help me to adapt it to allow for 3-tier dynamic population.
