Yes we have tried restarting the Cold Fusion application server service.
It is really weird, and like nothing I have seen before. Right what we have done is go back to the application pool hang, and added application pools to get the websites and app pools to a 1:1 basis.
We will monitor this for a while and see if this helps with the issue.

Have you tried restarting the Cold Fusion application server service instead of restarting the whole server?
This almost sounds like ColdFusion is hanging. If that's the case it could cause hung threads in the worker process which could lead to it exceeding shutdown timeouts during a recycle. Also, if it is hung then restarting the worker process is not going to cause the ColdFusion application server service to restart.
