Citrix PS4 Migration

I am in a preparation process of Citrix PS4 server migration for one of our client.

Current Environment
Total Farm =1
Servers =2 (Server1 & Server2)
Citrix Ver = PS4 Advance
DataStore = SQL Server
Citrix Users = AD Users
Client Access = Citrix Neighborhood, WI
OS Ver = Windows 2003 Ent
Hardware = 2 x IBM Servers

New Environment
Total Farm =1
Servers =2 (Server1 & Server2)
Citrix Ver = PS4 Advance
DataStore = SQL Server
Citrix Users = AD Users
Client Access = Citrix Neighborhood, WI
OS Ver = Windows 2003 STD
Hardware = 2 x BLADE Servers

Objective: Migrate to better Hardware, but need to keep the same Farm Name, IP Address and Citrix Version.

Server1 = Datastore, Citrix Lic Server, Web Interface, 15 Published Applications
Server2 = 21 Published Applications
Server3 = DC & TS Lic Server

All servers are in the same subnet, initial setup need to do in temporary server name, IP address and farm name. Need to change to current name and IP on the day of golive. I am approaching this setup first time. Below are my queries.

1.      What is the best approach for this project (steps of migration)
2.      Whether change of IP, Server name and Farm name can be possible? What will be the impact?
3.      How do I migrate License server and Data store
4.      How do I migrate all the published applications
5.      I found total 200+ printers in the server, but not locally installed. All auto created client printers. How do I configure this
6.      Currently load Manager (Load evaluator) configured, how do I configure in new setup or can migrate current setup.

Let me know if you are not clear of my scenario.
