How do I Upgrade from Citrix Presentation Server 4.5 to XenApp 5?

Here's the situation:  We have one single Citrix server running Presentation Server 4.5 without FP 1 and no Hotfix Rollup Packs.  I (a non-Citrix expert) have been given the job of bringing the server up to current.

I find the Citrix documentation hard to understand, so I wanted to get a critique of my plan.  

After a lot of reading, it looks to me like there really isn't a XenApp 5 installation routine?  The install files I have downloaded are all branded Presentation Server 4.5 when I run them on a test box.

The documents I've read all mention some prerequisites (Java to 1.6 update 5 or higher, Visual C++ Runtime) as if they need to be installed manually.  Is that true, or are those included in the Citrix install routines somewhere?

ECISCitrix server OS:  Windows 2003 SP2
License server:  4.5  (aka 11.3)
Citrix Presentation Server for Windows, Enterprise Edition
Presentation Server:  4.5 SP 2006.10
Feature Pack 1: Not installed
Hotfix details: No items to display

Proposed Upgrade Plan:
Upgrade Java to 1.6 update 5 or higher
Install Visual C++ 2005 runtime
Upgrade license server
Upgrade Access Management Console
Install Hotfix Rollup Pack 4

What am I missing?

Upgrade Java to latest version - Java 6 Update 16 IIRC.
Install Visual C++ 2005 runtime
Upgrade License Server to 11.6.1
Upgrade AMC
Install HRP4

You can install WI 5.1.2 if you like the black color scheme or WI 4.6.  I prefer 4.6 myself.
