I've been over all the obvious and am hitting a wall. Yahoo does not log external IP or anything helpful for that matter. It almost makes more sense this person is remoting in, but I just don't see how I could be missing it. All passwords have been changed. Problem persist. This one is deep, guys. Please help.

Hi,
My sincere apologies for not responding (somehow, I never saw your response until today).
The following is not for points, just trying to help if possible:
1. Are you on a wireless network? If you are using WEP encryption, it is possible for someone to break into your system and take control of the same.
2. Check whether you have any Remote PC Access Software installed in your machine. A list of such software is available here:http://en.wikipedia.o
Hope this regards,
Warm regards,

You should unplug the computer from the Internet run an good antivirus scanner and then try to install a firewall program eg: zonealarm or tinyfirewall and do a windows update. After this i think there is no way to access your computer and don't forget to change the old passwords.
