Can not add user for ESXi server (Standalone)

We have a stand alone ESXi server which we will be utilizing for a test environment.  It is not attached or managed by our Virtual Infrastructure 3 system.  

We are wanting to add a couple of users on the server other than the root user.  We use the Infrastructure client to connect to the ESXi host, goto 'Users and Groups', right click in the blank area, and click on add.  

It brings up with the window just fine, we type in a Login, a password yet the OK button is consistently greyed out.  No matter of filling in the UID, username, group memberships etc... will un-grey it.  I have combed through documentation from VMware and how they tell you to add users, we are duplicating exactly.  We have also done a reinstall of the ESXi server several times from several different sources (New Download, old download etc...)

This seems like a really silly problem to have as everything else but adding new users works for this ESXi server.  It's probably something very simple we're overlooking but would appreciate any assistance you have. Again, this is a standalone ESXi server, not connected through virtual center.

wow, that is beyond awful.  I must have spent 2 hours searching for a solution to this issue yesterday afternoon.  Appreciate your quick and thorough solution.
