difficulty getting firmware update applied to qlogic iscsi hba to become compatible with ESX 3.5

This all started a couple days ago when trying to install new servers to replace ESX boxes destroyed in indoor rain (note to others, do not adopt ceiling mounted ACs over your production rack)

Anyway, ESX 3.5 on two identical Dell 2950s. Identical Qlogic qle4060Es installed. Both hang on boot at loading vmkernel qla4022.o (options) after selecting boot ESX normally (not debug).

Some investigation and support calls to Vmware, Dell, and Qlogic said to update (if needed) bios and firmware to 1.13 and 3.0.1.33, respectively. Verified BIOS for HBA, I don't know the firmware rev.

I burned a bootable CD that allows me to change from A: to R: and then listed directory contents of R: (which is a Cd containing the DOS4GW, IFLASH, QL4022RM.bin and QLA4022.dl (fw update)). When I go to iflash(.exe) /ff (firmware instructions from qlogic's site), I get: Qlogic iscsi adapter not found. I know it's there because if I reboot, I can go into the BIOS of the card and view its information i.e. serial, bios version, etc.

I've been here before.  Try removing the HBAs from the server and putting them in a PC with a floppy drive one at a time. Create a bootable floppy with the BIOS and firmware on it and boot from that (you can safely remove extraneous stuff like the docs, KBD.SYS stuff and so on). The update BIOS makes a copy of the current BIOS and firmware then updates the HBA.

Thanks for the response, but i don't think I follow. I should have listed that these machines have PCI-E and I will be hard pressed to locate a PC with that interface. Perhaps the new Dell 755s do have them and for argument's sake, let's say they do...you directed to insert the card into the PC, go to the BIOS (of the mainboard or HBA?) and "create a bootable floppy" from there. Could you explain that process a little please?
Also, once created, I would reinsert the card into the server, (no floppy so would need to create a bootable CD using that floppy) and then carry on with the process by referencing the CD I created from Qlogic with firmware update?

First, leave the cards in your server for the time being.

Insert a floppy disk into the floppy disk drive, open My Computer in Windows XP or Windows Server 2003 and right click on the floppy drive. Select Format. Tick the box 'Create an MS-DOS startup disk'. Windows will create a bootable floppy. You can then remove unnecessary files such as KYBD.SYS and so on to make enough space for the QLogic firmware and BIOS. Copy the QLogic files to the floppy. Boot the server from the floppy and run the batch file (the title varies depending on the model of card). The update takes around 10 minutes.

Solution:
It was an incompatible IBM card from Qlogic (Dell ordered). The 4060C comes in several flavors. Ensure your rep (or you) order(s) the correct card and saves you many hours of tshooting.
