Problems after upgrade BackupExec 11.d to version 12.5

Weve recently upgraded the BackupExec 11.d to version 12.5 due to our server upgraded from windows 2003 (x64) to windows 2008 (x64).
The server running BackupExec is a domain controller with Windows 2008 x64.

Problems and symptoms:
There are errors popup every time I run the backup job. Then from the windows application event log I got the details of these errors as below:
Event ID: 34326
Access to catalog index (Catalog index database) failed.
Reason:  [Microsoft][ODBC SQL Server Driver][SQL Server]Procedure or function 'UpdateCatMedia' expects parameter '@PartitionID', which was not supplied. cat_RecordSet::Open()

Event ID: 34327
Update to catalog index (Catalog index database) failed.
Reason:  [Microsoft][ODBC SQL Server Driver][SQL Server]Violation of PRIMARY KEY constraint 'PK_CatResourceNameNode'. Cannot insert duplicate key in object 'dbo.CatResourceNameNode'. cat_RecordSet::ExecuteBulkInsert()

Event ID 34338:
Backup Exec Alert: Catalog Error
(Server: "SSFILE01") ODBC access error. Possible lost connection to database or unsuccessful access to catalog index in the database.

And even worse today I found that we can't restore data from the backup tape!
I got an error says the selected media was not found in the catalogs&. when I tried to restore data from an online media:
The selected media was not found in the catalogs. You must first catalog the media if you want to use it for the restore operations.

It sounds to me the media database is crashed and BE can't recognize the media.

I  have searched Symantec KB database and found this article:

I have tried the method it provided but that didnt fix my problem.

Any help would be much appreciated.
