If you use XP, just download it to the desktop and run it. It will ask to install the recovery console and you should do that in case you need to regain access to your drive.
In Vista, you need to run it as elevated administrator (right-click: run as administrator), same procedure.
It will scan for known rootkits and malware and normally all active malware will be deleted at the end of the scan. You can finish it up with a Malwarebytes' scan or a simple anti-virus scan to clean all other files which remain in the computer but aren't active
