Best route for dealing with malware

I've dealt with about 10 incidents recently where trojans have got past various levels of McAfee and Symantec antivirus/antispyware protection. All problems have been resolved by this forum and rpggamergirl in particular.

How the PCs get the trojans - I don't believe any of these users would have opened an email attachment from any unknown source, so am I right in thinking the only 2 ways on getting infected would have been through loading/downloading file or visiting an infected website. Would they needed to interact with the website in order to get infected? Nearly all the users who had problems have been girls/women under 35, so I don't think the sites they're visiting are too blue - is there a way my clients can check if a website is clean or infected before it?

Dealing with "infected" PCs - Once I am satisfied it is potentially a virus problem the course I have normally been taking is:
1. Run Malwarebytes to detect and repair.
2. Run ComboFix to detect and repair.
On a couple of occasions these 2 steps have got things back to normal, if not...
3. Run HiJackThis for a report and send off to HJT forum
4. Make deletions and run Kaspersky or Smitfraud as instructed.
I run all of these in Normal mode, connected to the Internet. When the infection is a little more developed and HiJackThis is blocked from running, even with a changed name, I run Deckards, maybe in Safe mode if needed, to get a HJThis log.
As I'm running CF, Mbytes, HJT, Kaspersky some could cancel others out - which are the best to rely on and what order should they take?
If leave Kaspersky on their systems and get them to run this once a week would this keep things under control? Is it advisable to get everyone to download and run Kaspersky anyway or are there trojan that this doesn't detect?
