Detailed steps for adding form fields to a .pdf file

Hello fellow experts

I now have Adobe Acrobat 7 Try-Out, with a .pdf file that I have created opened.  What are the detailed steps for add form fields to it?  Specifically, I have an Access report that is printed 200x for 200xish business units, and I'd like to distribute these with form fiends that these remote users can fill out and email back to me in a manner I can extract back into my Access database.

I was able to accomplish all of the above using Adobe Acrobat 7.0 Professional (Tryout Version).

Working in both Access and VB.NET.  Wooo hooo!!
BTW: If you want to assign a value to your form field, you can do this with this line after you create the field:
field.value = "some dummy text"
