if you want to create custom form, create your php code in a file, have the form in wp page, and the form post to the php page and then redirect again to a success page

How to create a PHP form inside wordpress page
Recently I have installed wordpress version 2.6 in my site, now I want to create a page that includes a php form to insert and retrieve data from a database, I know how to write the php code but I don't know how to integrate it with the wordpress page, Can any body help or give me a guidelines how to do it?

I have php file "form.php" it inserts and retrieves a data from a database "not wordpress database", I want to see this form inside my wordpress page and enable any visitor to use the form to inserts and retrieves the data he needs. wordpress enable you to create a pages, but how I can integrate these pages with "form.php" ??
Ok I will form my question in other way, before we install wordpress we used to have a web application developed in php, now after installing wordpress we want the visitor to access and use the php application through the wordpress, in other word we want to integrate the previous application with the current wordpress.

Ok on this link for example you will find contact form which is written in php, how you can place this form or integrate it on wordpress, so the user once he enters my blog he will find the contact form.
