ActiveX Trouble : Automation server can't create object.

Trying to teach myself how to embed an ActiveX control into an asp.net website.
I was following a tutorial found here: http://www.dreamincode.net/forums/showtopic38890.htm

The code I'm trying to work with is shown in the code snippet below.

I've tried loading the code via visual studio's virtual server and through IIS running from my local machine.  In both cases, the javascript would fire showing the "Running" alert, and then the browser would throw the following error:

Automation server can't create object.

Any ideas what I'm doing wrong?
