Could be a number of reasons - Quark 5.0 does this a lot when creating PDF's. In rare cases, you may be using a version of Acrobat prior to the one that that this particular pdf was created for. In that case, upgrading to a more recent version of Acrobat may help. If that is not the case, you will not be able to fix the error within the existing pdf.

Sounds like a corrupt PDF. Not much you can do about it except replace it with a good copy.
