The EMail button is part of the File toolbar. Which tools are visible is determined by the following registy keys:
HKEY_CURRENT_USER\Software
The order of the buttons depends on which ones are selected. When I just enabled the email button, it ended up being a2, but you can just add a new entry after the last one (e.g. the last one is a5, you would add "a6"="AcroSendMail:SendMai
