Problem solved - but thanks!
I printed from Acrobat to the Adobe PDF creator print driver.
The file went from 18Mb to 2MB - although I did have to recreate the bookmarks and links.

I have a 22 page PDF document with a mixture of text and graphics, compiled in Acrobat 7. When I save the file normally it's about 18MB. I have used the PDF optimizer with the following settings:
Images -> Bicubic downsample all images to 72 dpi using JPG medium
Fonts -> Three embedded subsets
Transparency -> Flatten, raster/vector 75, line art, text, gradient 150, convert text & strokes to outlines, clip complex, preserve overprint
Discard -> Discard alternate images
Clean Up -> Compress document structure, use Flate, optimize for fast web view
The end result is a file almost 7MB in size. My client wants to use this file for emailed responses to customer inquiries - we need to get this smaller.
The audit space usage report shows:
Thumbnails 9kb
Images 1,826kb
Bookmarks 2.6kb
Content streams 950kb
Fonts 276kb
Structure info 113kb
Acrobat forms .1kb
Link annotations 4kb
Comments .8kb
Doc Overhead 100kb
Color spaces 1,951kb
Extended Graphics .4kb
Piece information 1,436kb

You are using almost 2MB for color spaces - do you have ICC profiles in the file? If so, can you change the colorspaces to non-color-managed?

If you save it as not optimized for web view, the file will be smaller.
